Immunization Records Requests - Updated Information
If sites/programs are receiving calls or emails about needed changes to the client's immunization record, please ask the individuals to complete the online form. This will expedite the process to resolve any inquiries by ensuring the individual has provided all the information required to add or correct information.
This process can be used for situations where the individual received an immunization from out of province and now would like that dose entered into PHIMs or where there is inaccurate or missing information. Individuals are asked to allow a minimum of three (3) working days from the time of their immunization before submitting a form to allow time for the record to be entered and uploaded into the Shared Health Cord system.
Effective June 30, 2021 at 6 p.m., individuals without access to the e-form can call 1-844-MAN-VACC (1-844-626-8222) and follow the prompts to get their update process started.
