Occurrence Reporting Requirements
Applicable to : Acute care/Transitional Care Sites/Personal Care Homes
Follow-up/Update from previous Call to Action dated March 11, 2021.
The following update is to provide clarity on occurrence reporting completion related to COVID-19:
For any Patient/Resident who has a confirmed COVID-19 Health Care Acquired Infection deemed by Infection Prevention & Control an occurrence report is to be completed. The classification of the event is considered an occurrence. If the Patient/resident condition happens to deteriorate i.e.) such as admission to an ICU/extended hospital stay or death, then at that time would meet the threshold/criteria of a reportable critical incident.
Follow these next steps:
- Obtain the original occurrence report and document the patient/resident change in status by adding the status update on to the original occurrence report.
- The original occurrence report category of an occurrence can be modified from an occurrence to a critical incident at the originating site by the respective manager.
- Notify the Regional Lead - Quality, Patient Safety and Accreditation &/or the Regional Coordinator - Patient Safety as soon as possible and no later than 24 hours. If notification is on the weekend and not during regular business hours, please contact the Portage District General Hospital at 204-239-2211 and ask the Senior Leader On-Call to be paged in accordance with the regional Senior Leader On-Call policy (ORG.1010.PR.003).
- Scan the updated occurrence report to the Regional Lead - Quality, Patient Safety and Accreditation &/or the Regional Coordinator - Patient Safety
A Staff member who has a COVID-19 Health Care Acquired Infection that has been linked to an outbreak by Infection Prevention & Control requires an occurrence report to be completed. All community acquired COVID-19 infections do not require an occurrence report for staff&/or patients.
When an outbreak is declared in a facility and it equates to a disruption to the delivery of service and programs (unplanned or unexpected), it should be reported as a Critical Occurrence.
Leads/Directors (or delegates) review and share the reporting requirements with all applicable staff.
